NON-HEALING ULCER

—Tuberculosis is the usual diagnosis

A 35 years old male presented with a non-healing ulcer in the left flank for the last 14 months. He had multiple courses of antibiotics but there was no improvement and the ulcer kept on increasing in size. He presented with marked purulent discharge and extensive skin necrosis. His ESR was 35 mm after 1^st hour. The CBC, urinalysis, Chest X-Ray and ultrasound abdomen were normal. Wound debridement was performed under GA and tissue was sent for histopathology. This confirmed chronic caseating granulomatous inflammation suggestive of Tuberculosis-skin. He was started on anti-tuberculosis treatment and the wound healed completely in 4 weeks. The treatment continued for 6 months
